When mobster Paul Vitti suffers a crisis of confidence, he turns to professional help in the way of psychiatrist Dr. Ben Sobel. As Ben's world is turned upside down by Vitti's demands, the FBI approaches Ben with an offer he can't refuse. Complications escalate as Ben finds himself dodging bullets and rival gang members, but still wanting to help Vitti. It all comes to a climax when Ben must act as Vitti's consigliore to confront a rival gang chieftain.
