As the bubonic plague spreads through Italy, a group of nobles and servants retreats to a villa, where their lavish getaway quickly spirals into chaos.

In 1348, as the Black Death spreads across England, a young monk is sent on a mission to investigate rumors of a remote village unaffected by the plague. He soon discovers that the village has made a pact with a necromancer, who is using the plague to carry out acts of witchcraft. Faced with both the brutality of the plague and the terror of the supernatural, the monk must confront his own faith and beliefs to survive.

Suddenly appearing in Florence, an evil seductress causes Cesare, the city's ruler, and his son to both fall madly in love with her. The son, killing his father before an order to torture the woman can be carried out, then turns the city's churches into dens of sexual debauchery. Acts of evil and corruption continue unabated until the arrival of Death, who brings with her a horrible plague which she is about to loose upon the city.
