Chelios, a man with an artificial heart, seeks revenge on a Chinese mobster who stole his indestructible heart and replaced it with a battery-powered ticker. To keep himself alive, Chelios must deliver regular jolts of electricity. In his quest for vengeance, he battles various obstacles, including Mexican gangsters, abusive customers, and the leader of a triad. Along the way, he reunites with his ex-girlfriend and encounters a group of porn stars, cops, and a man who has his brother's head alive in a tank. Despite being set on fire, Chelios emerges victorious and gets his heart back.

Chev Chelios, a professional assassin, is injected with a poison that will kill him if his heart rate drops. To survive, he must keep his adrenaline pumping through constant excitement and danger. With only a limited time left, Chelios sets out on a mission of revenge against his rival, Verona, while also trying to protect his girlfriend. The film takes place in a single day filled with intense action, fight scenes, and car chases. In a final showdown, Chelios confronts Verona and defeats him, but ultimately sacrifices his own life for the sake of his mission.
